are excited for your stay at Martha's Place,
and will strive to provide a fun and
relaxing atmosphere for you! In
order to best serve you, please read, print,
and complete all of the following important
documents to help us get better acquainted with
one another:
VIEWING
INSTRUCTIONS:
 You
will need Adobe Acrobat Reader to view PDF
files. If you don't already have it, you can
download it for free at www.adobe.com.
These forms must
be completed, signed, and returned to Quiet
Waters Outreach at least two weeks prior to your
stay (unless other arrangements are approved
by QWO's management) in order to confirm
your reservation.
We require
all future guests to tour Martha's Place before your stay
so that you can familiarize yourself with
the surroundings and our staff members.
