We are excited for you to participate in our Discovery Day Program, and will strive to provide a fun and enjoyable atmosphere!  In order to best serve you, please read, print, and complete all of the following important documents to help us get better acquainted with one another:

• "No Violence" Acknowledgement

You will need Adobe Acrobat Reader to view PDF files. You can download it for free at www.adobe.com.
 

These forms must be completed, signed, and returned to Quiet Waters Outreach at least two weeks prior to your first day at the program (unless other arrangements are approved by QWO's management) in order to confirm your reservation.  

We require all future guests to tour our facilities before your stay so that you can familiarize yourself with the surroundings and our staff members.
